Set up a clean field
Wash your hands. Pull a fresh sheet of paper towel onto a clean surface and set out everything before you start: the lyophilized vial, the BAC water, alcohol pads, fresh sterile insulin syringes, and your sharps container.
The single biggest cause of contamination is touching the bevel of a needle or the rubber stopper of a vial after the alcohol has dried but before the draw. Slow down.
Caution: Do not skip the sharps container. Handing yourself an exposed needle is how needlesticks happen.
